ATTENDANCE FORM - 2025-2026

In order to obtain a valid physician’s excuse, students must be seen by a physician in his or her office or hospital. Telephone excuses will not be accepted. Medical excuses will not be included when considering disciplinary action and/or loss of privileges due to excessive absences.
